All the 'black swans" without exception you mentioned were predictable or even predicted.

So the real question is: why were they not acted upon.

It's because our societies are not run by our best and brightest but by extremely mediocre power-hungry people. This is possible because we use a system of governance that is severely flawed. And if we don't want it to kill us we will have to change it. But I doubt that will happen in time.

So until then, enjoy and brace for impact.
